What are Jaw Joint Disorders?

Temporomandibular Disorders (Temporomandibular Joint Disorders – TMD) refers to the general term for anatomical, functional, or inflammatory disorders occurring in the joint structure where the lower jaw connects to the skull base, including the cartilage disk of this joint and the surrounding chewing muscles.

The temporomandibular joint has one of the most complex movement mechanisms in human anatomy. This complex structure, which supports vertical and horizontal movements such as opening, closing, and sliding left and right of the mouth, consists of the head of the lower jaw (condyle), the joint cavity in the temporal bone, a cartilage disk that serves as a cushioning pad between these two bony tissues, and the working chewing muscles attached to the joint. A disruption in the harmony between these parts causes temporomandibular discomforts.

At the Avrupadent clinics located in İzmir, temporomandibular discomforts are evaluated under various clinical scenarios such as the displacement of the intra-articular cartilage disk, excessive tension (spasm) in muscles, and wear of the joint surfaces. This condition directly affects essential actions in daily life such as speaking, eating, and yawning and is a health issue that requires re-establishing physiological balance.

⚡
Intra-Articular Biomechanical Balance
The articular disc located within the jaw joint prevents the direct collision of bone surfaces. The displacement of this disc from its anatomical position is a key factor that leads to the formation of grinding noises when the mouth is opened and closed, and to the sensation of the jaw being stuck.

What Causes Jaw Joint Discomfort?

Causes of Jaw Joint Discomfort; is a combination of teeth grinding and clenching (bruxism) during sleep or while awake, postural disorders, occlusal discrepancies in teeth alignment, physical trauma to the jaw area, and stress factors.

Jaw joint problems generally develop as a result of multiple factors coming together (in current medical terminology, this is referred to as multifactorial). The most common factor encountered in today’s fast-paced lifestyle is the individual unconsciously clenching or grinding their teeth. This action exerts stress from the chewing muscles onto the joint capsule, causing micro-trauma to the tissues.

Additionally, uneven contact between upper and lower teeth due to missing teeth or faulty restorations (occlusal disorders) causes the head of the joint to slip within the socket each time the jaw is closed. Over time, this uneven load distribution disrupts the structure of the cartilage inside the joint.

Reason GroupImpact Mechanism on the Jaw Joint
Bruxism (Teeth Grinding / Clenching)Creates continuous tension in the muscles, applying excessive pressure to the joint capsule and shifting the disc forward.
Occlusal Discrepancies (Malocclusion)Asymmetrical positioning of the jawbone head within the joint socket due to incorrect contact between teeth.
Macro and Micro TraumasTissue damage from direct impacts to the jaw or habits such as prolonged chewing of gum.
Posture and Position ErrorsImbalance in tension in the neck and jaw muscles due to the head constantly being in a forward-leaning position.

What Are the Symptoms of Jaw Joint Problems?

Symptoms of Jaw Joint Problems; clicking or popping sounds from the joint when opening and closing the mouth, limited jaw movement, radiating pains in front of the ears, headaches in the temporal region, and jaw locking.

Symptoms may initially present at a mild level and can fluctuate over time. The most common early sign is a clicking or popping sound coming from the area just in front of the earlobe while eating or yawning. This sound occurs when the cartilage disc abruptly settles into place while the jaw head is moving.

As discomfort increases, discomfort can also be seen in radiating areas. The main clinical symptoms are as follows:

    \t

  • •
Restricted Jaw Opening (Trismus): Inadequate mouth opening while yawning or eating or the jaw moving to one side while opening.
    \t

  • •
Forehead and Jaw Pain: Tension-type pain that spreads to the forehead, cheeks, and neck, related to fatigue of the chewing muscles.
  • •
Jaw Locking: Sudden locking of the jaw when open or closed, requiring manual intervention.
  • •
Ear Sensitivities: A feeling of fullness or ringing in the ear due to an inflammatory condition near the joint area affecting the ear canal.

How are Jaw Joint Disorders Diagnosed?

Jaw Joint Diagnosis; involves taking a detailed patient history, manual muscle and joint examination (palpation), measurements of millimetric mouth opening, panoramic X-ray, 3D tomography, and Magnetic Resonance (MR) imaging showing cartilage structure.

The assessment process begins with the physician examining the joint area and the chewing muscles with their fingers from the outside. During this examination, trigger points of pain in the muscles and shifts during joint movements are detected. The patient’s maximum mouth opening is measured with a ruler (normal values range between 40-50 mm).

In order to examine the condition of the hard tissues (bone surfaces), three-dimensional dental tomography is utilized; while the position, deformation of the cartilage disc and joint fluid increases are analyzed with MR imaging that provides details of soft tissues. The obtained data is synthesized to clarify the diagnosis.

How Is Jaw Joint Treatment Performed?

Jaw Joint Treatment; it starts with protective (conservative) methods based on the stage of the disease, using personalized night splints, muscle relaxant medical protocols, joint lavage (arthrocentesis), and gradually advancing with physiotherapy exercises.

At Avrupadent clinics, jaw joint treatment is structured with preventive measures before irreversible procedures. In the initial phase, hard acrylic occlusal splints (night splints) are applied to reduce the pressure that teeth exert on each other and the stress in the joint capsule.

In cases of severe muscle tension in chewing muscles, relaxing medical applications targeting the masseter muscle may be preferred. In later stages, procedures based on sterile fluid injections may be performed to clear intra-articular formations.

Why Are Jaw Joint Disorders Important?

When jaw joint issues are postponed, residual tears in the cartilage tissue, calcification on bone surfaces (osteoarthritis), and lock-ups that make mouth opening impossible may develop, hence it is important to manage them early on.

Initially, a slight clicking sound when yawning may gradually turn into severe chronic pain due to the wear of intra-articular tissues over time. Deformations in the joint structure can hinder chewing function, negatively affecting an individual’s diet and speech quality.

Evaluations conducted at Avrupadent centers in Izmir aim to prevent these progressive tissue damages and maintain the balance of the biomechanical system.

Who Experiences Jaw Joint Problems?

Temporomandibular joint disorders can occur across all age groups; however, they are observed more frequently among adults aged 20 to 50, individuals under high stress, and women.

The reasons why these disorders are seen more frequently in women than in men include hormonal factors and differences in connective tissue flexibility (connective tissue laxity). Additionally, those who work at desks, individuals with posture disorders, students during intense exam periods, and those with teeth grinding habits are more predisposed to this discomfort.

What Treatment Methods Are Used for Temporomandibular Joint Disorders?

In the treatment of joint disorders, various clinical methodologies can be applied simultaneously, targeting the underlying cause of the disease:

Treatment MethodFunction and Mechanism of Action
Occlusal Splint (Night Plate)Reduces vertical pressure inside the joint by restricting tooth contacts, directing the cartilage disc to the correct position.
Masseter Muscle TherapiesTemporarily alleviates the activities of overstressed chewing muscles, reducing the strain on the joint.
Arthrocentesis (Joint Washout)Removes inflammatory substances in the joint capsule, opens adhesions, and delivers a lubricating solution.
Physical Therapy and MedicationsRegional exercises along with muscle relaxants and hot/cold compress applications.

What Are the Benefits of Jaw Joint Treatment?

The basic benefits of temporomandibular joint treatment are; the possibility of comfortable chewing and speaking, relief from headaches/neck pain, halting tooth grinding, and preservation of joint structure.

As chronic tension in the muscles decreases due to treatments, pain radiating to the temples and around the ears is brought under control. The risk of cracks and wear on the teeth that develop due to teeth grinding is eliminated.

How Do Temporomandibular Joint Disorders Affect Dental Health?

Temporomandibular joint issues are directly related to dental health. The uneven loads imposed on the joint usually lead to teeth grinding, which erodes and flattens the protective enamel layer on the chewing surfaces of the teeth over time.

In worn teeth, sensitivity to hot and cold increases. Additionally, excessive pressure can cause notches (abfractions) in the neck areas of the teeth, resulting in fillings and crowns frequently breaking or getting damaged.

Do Temporomandibular Joint Issues Cause Jaw Pain?

Yes, problems with the jaw joint can cause significant pain in the jawbone, temples, and in front of the ears, due to lactic acid accumulation in the muscles and increased pressure in the joint capsule.

Continued contraction of the chewing muscles weakens blood circulation and causes muscle spasms. These spasms trigger tension-type pains that spread not only to local areas but also to the sides of the head and neck muscles.

What Does It Mean When There Is Noise in Jaw Joint Disorders?

The clicking or popping sound coming from the jaw indicates that the cartilage disc has shifted forward from its anatomical position and that the bony head is hitting this disc during jaw movement.

A clicking sound is an early indicator of joint incompatibility. A change in the sound over time to a crackling or grinding form (crepitation) indicates that the cartilage tissue has thinned and that the bony surfaces have started to come into contact with each other.

What Causes Jaw Locking?

Jaw locking occurs when the disc of the cartilage completely blocks the movement path of the bone head. It can manifest in two different ways:

    \t

  • Inability to Open the Mouth (Closed Lock): The disc moves forward, preventing the jawbone from moving forward.
  • \t

  • Inability to Close the Mouth (Open Lock): During stretching, the jawbone head slips out of the socket and gets stuck.

When Should You Seek Specialist Support for Jaw Joint Discomfort?

It is recommended to evaluate if any of the following warning clinical conditions are noticed:

    \t

  • Regular popping or cracking sounds coming from the jaw when opening the mouth.
  • \t

  • Feeling of fatigue, stiffness, or pain in the jaw muscles in the mornings.
  • \t

  • The jaw getting stuck or slipping open while eating.
  • \t

  • Experiencing attacks of open or closed lock in the jaw.

How Long Does Jaw Joint Treatment Take?

The duration of treatment varies depending on the extent of tissue damage in the case. Simple muscle spasms can be controlled with a few weeks of medical follow-up, while splint (night guard) treatments typically last between 3 to 6 months.

In Avrupadent clinics, patients are called for check-ups at specific intervals for the adjustment of plates, and the healing progress of the joint is monitored.

What Should Be Considered After Jaw Joint Treatment?

After treatment, it is essential to regularly use the prescribed plates, take small bites, avoid chewing gum, and support the jaw from below with the hand while yawning.

It takes time for the joint tissues to recover. One should avoid chewing hard foods with the front teeth (such as apples, quinces, etc.), nail-biting, and resting the jaw in the palm.

Can Jaw Discomfort Recur?

An increase in stress levels or the cessation of using protective plates can trigger teeth grinding habits again and symptoms may resurface. Therefore, periodic check-ups should be conducted.

What Should Be Considered to Protect Jaw Health?

  • • Balanced Chewing: Ensure to use both sides equally while eating instead of favoring just one side.
  • • Stress Management: Allocate time for breathing and stretching exercises to reduce tension throughout the day.
  • • Completing Missing Teeth: To maintain bite balance, replace missing teeth with prosthetics/implants.
